            On this 12th day of February in the year of our Lord one thousand eight hundred and thirty three personally appeared before the Court of Common Pleas held in and for the County of Rensselaer and State of New York, David Henry a resident of the Town of Lansingburgh in the County and State aforesaid, aged eighty two years who being duly sworn according to law doth on his oath make the following declaration in order to obtain the benefit of the provisions made by the Act of Congress passed June 7, 1832. That he entered in the Army of the United States in the year 1775 in Colonel Holmes Regiment of the New York State Troops under the following named officers. Says that he in the year 1775 entered in Sing Sing in the County of WestChester in the State of New York that in the month of May in that year at White Plains in the County last aforesaid he was enlisted a private soldier by Lieutenant Isaac VanWart into the Company Commanded by Captain Ambrose Horton belonging to Colonel Holmes Regiment of the New York State Troops for the Term of nine months that immediately on his Enlistment he joined the Regiment at White Plains, the latter part of the same month he embarked with his company on board of a sloop at TarryTown and came up the North River arrived at Albany the first of June following they remained in Albany about one month when he marched with his company to Fort George about 60 miles North of the City of Albany and [?] then to Ticonderoga where they were stationed until some times in the month of October following then by order of General Schuyler he embarked with his company for St. Johns in Canada when they arrived about the first of November after that on the 15th of the same month he was appointed (by Derick Swart the Deputy Commissary General for the Garrison of the Canada department) and issuing Commissary for the Garrison at St. Johns he served in that office until the following May when General Swart left Canada and returned home and Henry Benson succeeded him in that office he at this time served a renewal of his appointment to said office from the said Henry Benson and continued in the discharge of the duties of that office until the first of June following when he returned down to Sing Sing he place of residence aforesaid the reason of his [continuing?] in the service after the term of his first enlistment had expired was his having received the appointment above mentioned. That the whole Term of the above mentioned service was one year and one month, that all the pay he received for the above mentioned service was ten dollars which John N. Bleeker paid him on account which was indorsed on the back of the appointment he received from Henry Benson which he has long since lost.
            That immediately on his arrival at Sing Sing his place of residence in the month of June 1776 he enlisted into Captain [Ladnes?] Company belonging to Colonel Holmes Regiment of the New York State Troops to serve until the first day of January following that soon after he enlisted he was appointed orderly Sergeant for the said Company of Captain [Ladnes?] and served as such orderly Sergeant until he was discharged the first of January 1777 after having served seven months. That during this term of service he was first stationed with his Regiment at Fort Independence after remaining there a while he was marched with his Regiment to White Plains he was in the battle of White Plains under General George Clinton that soon after the battle was over he was marched with his Regiment to Peekskill where they were stationed until the first of January 1777 when he was discharged as above stated and returned home to Sing Sing aforesaid where he resided a short time after which he lived in different places in the State of New York until he moved to Lansingburgh in the County of Rensselaer and State aforesaid which was about forty six years ago where he has resided ever since and continues to reside there.
            He hereby relinquishes every claim whatever to a Pension or Annuity except the present and he declares that his name is not on the Pension roll of any agency in any State or if any only on that of the agency in the State of New York. (Signed) David Henry
